At Affinity Medical Home, you are the center of your health care team, which is led by your provider. Together, we will work with you to create a personalized care plan to manage your overall health and enhance your quality of life. For you, this means you will notice a more personalized experience when you visit with staff who know your medical history and specific needs. Affinity Medical Home provides access to quality care in a comprehensive team environment.
Summary of Affinity’s Journey
We began our medical home journey by introducing the concept to two of our primary care clinics in Kaukauna and Oshkosh in early 2009. Driven by the Affinity promise of delivering personalized care to our patients, the medical home model is now being rolled out to all of our primary care clinics.
The care team at the Affinity Medical Home consists of providers partnered with nurse practitioners or physician assistants, nurse specialists who coordinate ongoing health needs, behavioral health providers, patient service representatives and health care associates such as LPN or certified medical assistants. Other specialties, such as OBGYN or integrative medicine, may also be incorporated into the medical home model.
